Sliding Scale Policy

At the Colorado Center for Assessment and Counseling, we want to help everyone with their mental health, no matter how much money they have. We have a program called the Sliding Fee Discount Program, which gives discounted care to those who can't afford it or don't have enough insurance. If you can't afford health insurance or your current plan doesn't cover mental health services, you can ask to join the program. We will look at your income and family size to see if you qualify for a lower fee.

*Please note that we will require proof of income and family size, and the Federal Poverty Guidelines (http://aspe.hhs.gov/poverty) are used in creating and annually updating the sliding fee schedule (SFS) to determine eligibility.

To apply, you need to fill out an application form and provide proof of your income and family size. We define family as the head of the household, their spouse, and dependent children. Examples of accepted documents for income proof include tax forms, pay stubs, or letters from your employer. If you're self-employed, you'll need to provide information about your business income and expenses. In special circumstances, like if you're homeless, you may be able to provide a signed statement instead of written verification.

Once you're approved for the program, you'll receive a written notice by email with the percentage of the discount you qualify for. If you're not approved for a 100% discount or if your application is denied, you'll need to make payment arrangements with us. You can reapply after 12 months or if there's a significant change in your family income.

If you have trouble paying for our services and you refuse to pay or leave without paying, we will contact you in writing about your payment obligations. We may offer a payment plan or refer you for collections if you don't make an effort to pay or respond within 60 days.
